Welfare and housing support

Hospital to Home Welfare Advisers provide information and advice for the benefit of any patient on the pathway.

The team provides welfare support over the telephone and via email to the clinical teams to help them negotiate any welfare problem a patient or patient’s family are experiencing during their hospital admission.

In certain areas we also provide face-to-face assessment of welfare needs directly with patients and/or their family at the point they are referred to the service.

Families are supported with problems they may face during a close family member’s admission to hospital including:

  • Finding financial assistance available to cover unexpected expenses including transport and childcare costs.
  • Provide information on employment rights if a family member has to take a break or give up work.
  • Help families identify what help they can get to manage household expenses and housing costs whilst a close family member is in hospital and/or if they are unable to work.
  • Provide information to families about the local area if having to move to a hospital in an unfamiliar area.  This help could include finding suitable short-term accommodation.
