Hospital to Home Welfare Advisers provide information and advice for the benefit of any patient on the pathway.
The team provides welfare support over the telephone and via email to the clinical teams to help them negotiate any welfare problem a patient or patient’s family are experiencing during their hospital admission.
In certain areas we also provide face-to-face assessment of welfare needs directly with patients and/or their family at the point they are referred to the service.
