Only the HKW for the centre will see the patient for that centre until other team members have been linked. To link your team so they can see your patients, you need to make a request click here to the Pathway Support Team detailing who you want linked and they will do this for you.
Yes there are training modules and tools designed to guide you through the pathway which include helpful tips to get the most out of using it. The training will be in person with one of our dedicated trainers and a follow up remote service will be available also to answer any further queries you may have.
As well as the training, you will have access to a dedicated Pathway Support Team who will be able to provide you with guidance, advice and technical support.
The information held in the pathway will be confidential and will be available only to those you want to see it. It highlights and tracks all of the tasks associated with discharging the patients safely and there are mechanisms built in to ensure these are carried out in a timely and structured way so nothing gets missed. The MDT functionality concentrates on these tasks in particular and keeps meetings focussed on finding solutions to facilitate a speedy yet safe discharge.
