Hospital to Home’s Welfare Advisers provide welfare information and advice for the benefit of any patient on the pathway.
The team provides welfare support over the telephone and via email to the clinical teams to help them negotiate any welfare problems that are impacting a patient’s discharge from hospital.
In certain areas we also provide face-to-face assessment of welfare needs directly with patients at the point they are referred to the service and support throughout the discharge if required.
The focus of the support provided by the welfare team is to negate any unnecessary delays caused by suitability or availability of housing and other non-clinical problems that cause delay to a patient’s hospital discharge.
